6.3mm to 3.5mm Adapter Plug

Hi

This has probably been asked before, but where can I get a quality 6.3mm plug to 3.5mm 4-way socket adapter?

The phones (B&W P7) have a 4-way 3.5mm jack (includes mic).  If I use a standard 6.3mm plug to 3.5mm socket adapter, I get only one channel making contact.

My amp has the (standard) 3-way 6.3mm socket.
